It enrolled 7,447 high-risk adults in Spain and found that a Mediterranean diet supplemented with extra-virgin olive oil (4+ tablespoons/day) or mixed nuts reduced major cardiovascular events (heart attack, stroke, cardiovascular death) by 30% compared to a low-fat diet control — without caloric restriction.
The Mediterranean dietary pattern is characterized by: high intake of olive oil, vegetables, legumes, whole grains, nuts, and fish; moderate intake of wine with meals; low intake of red meat, processed foods, and refined carbohydrates. Critically, it is not a low-fat diet — approximately 35–40% of calories come from fat, primarily unsaturated sources.

For women specifically, the evidence is particularly compelling. A subgroup analysis found that Mediterranean diet adherence was associated with 35% lower risk of cardiovascular events in postmenopausal women.
