The gut microbiome โ the complex community of approximately 38 trillion bacteria, fungi, and viruses inhabiting the human digestive tract โ has emerged as a critical regulator of systemic health and aging. A landmark 2023 review in Nature Reviews Gastroenterology & Hepatology identified gut dysbiosis (microbial imbalance) as a key driver of the chronic low-grade inflammation that characterizes aging, now termed 'inflammaging.'
Centenarian studies have consistently found that long-lived individuals share a distinct gut microbiome signature: high microbial diversity, abundance of short-chain fatty acid (SCFA)-producing bacteria such as Akkermansia muciniphila and Bifidobacterium, and low levels of pro-inflammatory species. The gut microbiomes of centenarians in Sardinia, Okinawa, and South Korea show remarkable convergence despite dietary differences.

Dietary fiber is the primary fuel for beneficial gut bacteria and SCFA production. Fermented foods โ Greek yogurt, kefir, kimchi โ have been shown in Stanford clinical trials to increase microbiome diversity and reduce inflammatory markers (IL-6, IL-12p70) more effectively than a high-fiber diet alone after 10 weeks.
Refined sugar and ultra-processed foods alter the gut microbiome within 24 hours of consumption, reducing beneficial bacteria and increasing gram-negative endotoxin-producing species. This endotoxemia is a direct pathway from diet to systemic inflammation โ and from systemic inflammation to accelerated aging.
