Iron, B12 & Nutritional Deficiencies in Midlife Women
Two of the most common and consequential deficiencies in women over 40 are iron and vitamin B12 — and both are routinely missed in standard blood panels.

Iron deficiency without anemia — a state in which serum ferritin is low but hemoglobin remains normal — affects approximately 20–25% of premenopausal women and is associated with fatigue, reduced exercise tolerance, impaired cognitive function, and hair loss, even in the absence of clinical anemia. Standard blood panels that report only hemoglobin and hematocrit will miss this condition entirely; serum ferritin is required for accurate assessment.
Vitamin B12 deficiency is particularly prevalent in women over 50 who use proton pump inhibitors (which impair B12 absorption), have undergone gastric surgery, follow plant-based diets without supplementation, or carry polymorphisms in the MTHFR or TCN2 genes that affect B12 transport. A 2023 review in the British Journal of Nutrition estimated that 20% of adults over 60 have suboptimal B12 status — defined as serum B12 below 200 pmol/L — with neurological consequences ranging from peripheral neuropathy to cognitive impairment.

The dietary sources of B12 are exclusively animal-derived: liver, beef, salmon, sardines, eggs, and dairy products contain the highest concentrations. Women following plant-based or dairy-free diets require supplementation — methylcobalamin (the active form) at 500–1000 mcg/day is the most bioavailable oral form. Cyanocobalamin, the synthetic form used in most supplements, requires conversion steps that may be impaired in individuals with MTHFR polymorphisms.
